I accept and agree to the terms and conditions upheld by the Two52 Conference and Family Christian Center. *
Please read this form carefully and be aware that in signing up to participate in any activities or programs sponsored by FAMILY CHRISTIAN CENTER and/or 252 CONFERENCE, you will be waiving and releasing all claims for injuries arising out of these activities that you or the other named participants might sustain. The terms "I", "me", and "my" also refer to parents or guardians as well as participants in the activities and programs. In registering for these activities, you are agreeing as follows:
WAIVER, RELEASE OF ALL CLAIMS AND HOLD HARMLESS AGREEMENT
As a participant in these activities, I recognize and acknowledge that there are certain risks of physical injury, and I agree to assume the full risk of any injury(ies), damage(s) or loss(es) which I may sustain as a result of participating, in any manner, in any and all activities connected with or associated with such activities or programs. I further recognize and acknowledge that all unruly behavior with other participants is potentially hazardous and involves substantial risk, and may result in injury. I agree to waive and relinquish any and all claims I may have against FAMILY CHRISTIAN CENTER and/or 252 CONFERENCE as a result of participating in these activities, as well as any and all other persons and entities, of whatever nature, that might be directly or indirectly liable for any injuries that I might sustain while participating in these activities or programs. (The parties described in the preceding sentence are referred to as "released parties" in the remainder of this agreement.)
I do hereby fully release and discharge FAMILY CHRISTIAN CENTER and/or 252 CONFERENCE of any and all claims for injury(ies), damage(s) or loss(es) which I may have or which may occur to me on account of my participation in these activities or programs.
I further agree to indemnify, hold harmless and defend FAMILY CHRISTIAN CENTER and/or 252 CONFERENCE and the other released parties from any and all claims for injury(ies), damage(s) or loss(es) which I may have or which may occur to me on account of my participation in these activities or programs.
I further understand and agree that the terms such as "participation," "programs," and "activities" referred to in this agreement include all exercises and physical movements of any nature while I am participating in the programs and further include the provision of or failure to provide proper instructions or supervision, the use of any and all machinery, equipment, and anything related to my use of the services, facilities, or premises involved in these activities or programs, and transportation to any or from any event.
I understand the nature of the activities and programs for which I am registering, and have read and fully understand this WAIVER, RELEASE AND HOLD HARMLESS AGREEMENT. I further understand that any advisement or warnings of particular risks of the activities or programs that I subsequently receive will be incorporated by reference into and become a part of this agreement.
I hereby release FAMILY CHRISTIAN CENTER and/or 252 CONFERENCE of any responsibility or injury(ies), damage(s) or loss(es) to anyone for whom I am legal guardian or parent that may not be specifically named.
I give permission to Family Christian Center to give emergency medical treatment by medical personnel, hospital and/or medical staff, if needed, to me. I further indemnify and hold FCC and 252 Youth Culture harmless against claims, damages, injuries, or losses arising out of the activities that will be taking place during this conference.
